Legal Process : Medical information

In order to prepare the medical report, Swift may need access to your GP and/or hospital medical records. You will be required to sign a legal consent form to release the records.

For your information, you are entitled to obtain a copy of your medical records for your own perusal under the Access to Health Records Act 1990. You will usually be charged a nominal fee.
